When to use it
You meet it in documents, notices, meetings and work email; with friends, について is the natural choice.
Meaning
「〜に関して」means the same as 「〜について」but sits at a different level — written and formal: 「ビザに関して、質問があります」= I have a question regarding the visa.
How to attach
Before a noun it becomes 「〜に関する N」(「安全に関する規則」= rules regarding safety) — note that no の is needed, and this is exactly what the exam asks.

I have a question regarding the visa.
