N を くださいます (N o kudasaimasu) — A higher person gives something to me
A boss or a teacher gave something to me — the respectful version of くれます.

「くださいます」says that a higher person gave something to me (or to my family or my team). The giver is marked with が or は, I am 私に (usually dropped), and the thing given takes を. The base verb is くださる, but its ます form is not ×くださります — it is ○くださいます, and the past is くださいました. The direction is always "from another person toward me", so never use it for something I gave. The everyday 「〜て ください」comes from this same くださる. Which to pick? If the favour comes from a higher person toward me, use くださいます; if it comes from a friend or an equal, use くれます.
