〜と 読みます／書いて あります (~to yomimasu / kaite arimasu) — It reads ~ / it is written ~
Three patterns for reading something written, reporting it, and saying what it means.
Formula (सूत्र ढाँचा)
Grammar rule (व्याकरण नियम र परिभाषा)
と marks "quoting it like this", so whatever is written or said goes in front of と exactly as it is. 「〜と 読みます」gives the reading: この 漢字は 「みず」と 読みます. 「〜と 書いて あります」says something is written somewhere: ドアに 「押す」と 書いて あります. To ask or state a meaning, use 「X は Y と いう 意味です」: 「危険」は 「危ない」と いう 意味です. It must be と, not を or に — that is the most common mistake. When you meet a new 漢字 or sign, both 「これは 何と 読みますか」and 「これは どういう 意味ですか」are useful.
